A file in the scenario "The Hive" says that floors 4F and B3F of the hospital can only be accessed through a number code and voice print. This is exactly how Carlos accesses those floors in Resident Evil 3: Nemesis.


In Europe, the publisher decided to release this game without the possibility to play online.


During the Hive scenario, if Jim examines Dr. Hursh's body after he's defeated as the Leech Man in the Fixed Temperature Laboratory, he'll say it's 'That Dhalsim looking guy,' referring to the fighter of the same name from Street Fighter II, which is another game produced by Capcom.


More than 18 scenarios were originally planned for Resident Evil Outbreak, but most of them were cut, along with other additional content, or relegated to the standalone expansion Outbreak File #2. However, some of the scrapped character models can still be found in the game's data and are usable through the use of Gameshark or other such cheat device.


The red title font of the game is the same as the one from the Milla Resident Evil movies.


The name of the third scenario, The Hive, comes from the name of the underground Umbrella lab in the first Resident Evil movie.


This is the first Resident Evil game to feature online play.


Capcom canceled the internet servers for the game in the USA on December 31, 2007, and on June 30, 2011 in Japan.


Each scenario had a different designer.


The game revisits areas found in other Resident Evil games.  Below Freezing Point is the same lab from Resident Evil 2; and the hospital from Resident Evil 3: Nemesis is called The Hive.


Main areas in the game to explore are: Raccoon City streets, a bar, the Umbrella Lab, a hospital, Apple Inn, and Raccoon City University.


Version 1.0 of the US version has a game breaking bug involving Alyssa in the Outbreak Scenario. If Alyssa uses an Ad-lib in the alleys behind the Apartments, the game will freeze.


Mistakes


During the opening cinematic, while Hunk's team is being decimated by Birkin, a commando is shown cowering backwards in fear while firing his MP5. On top of the weapon there is a form of red dot sight, possibly an aimpoint. When Birkin impales the hapless commando and the gun goes flying, the scene cuts away to a shot of the gun hitting the ground in slow motion while still firing. In this shot, the scope/red dot sight is no longer present.


The game takes place in September 1998. However, a 2002 BMW can be seen in the first level.


In the scenario Decisions, Decisions, the UBCS Sergeant Nicholai is seen performing various objectives around the Raccoon University area up until the countdown for the nuclear missile strike to destroy the town starts. However, at the same time, Nicholai was trying to kill Jill at the dead factory just nearing the edge of town; even if he survived in the end, he could not have been at two places at the same time due to the countdown.
